Importance of Client Transparency Clients are encouraged to read these terms carefully to ensure understanding and informed consent. Legal Disclaimer - By using our booking services, clients agree to comply with these terms and understand the associated risks related to skin care treatments.Services Offered: 1. Facials 2. Microdermabrasion 3. Chemical Peels 4. Waxing 5. Brow and Lash Services.Contraindications - Clients must disclose any medical conditions or medications that may affect their eligibility for skin care services. Specific Contraindications for Each Service. 1. Facials: Allergies to ingredients, active skin infections. 2. Microdermabrasion: Eczema, rosacea, recent surgeries. 3. Chemical Peels: Use of retinoids within 24 hours prior to your service, skin sensitivity. 4. Waxing: Sunburn, skin irritations, allergic reactions to wax, contagious diseases. 5. Brow and Lash Services: Allergic reactions to dyes or adhesives. Client Responsibilities - Disclosure of Medical History, Clients must provide accurate and complete medical history regarding your service during the consultation. Honesty in Consent Forms, All clients are required to sign consent forms acknowledging the risks and benefits of the treatments. Understanding of Risks, Clients should fully understand the potential risks associated with the chosen skin care services. Potential Side Effects, General Side Effects / Common side effects may include redness, irritation, and sensitivity. Specific Side Effects 1. Facials: redness, breakouts, & adverse reactions. 2. Microdermabrasion: dryness, flaking, hyperpigmentation. 3. Chemical Peels: peeling, sensitivity, hypopigmentation. 4. Waxing: irritation, ingrown hairs, redness, hives, skin tears, flaking. 5. Brow and Lash Services: irritation, allergic reactions, damage to natural lashes. Booking and Cancellation Policy: Advance Booking Requirements Clients should book services at least 24 hours in advance. A minimum of 24 hours' notice is required for cancellations to avoid a cancellation fee resulting in a full service charge. Changes to Appointments: Clients may reschedule services up to 24 hours before their appointment. Liability Waiver: Understanding of Risks, Clients acknowledge that they have read and understood the risks involved in skin care treatments. Release of Liability, Clients agree to release the spa from any liability when booking a service for adverse effects resulting from their treatments.
